29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of walking/working platforms: a) 222 Route 22 West, Green Brook, NJ 08812. Employees were exposed to fall hazards when working on and from Tubular welded scaffold with no fall protection systems in place, i.e. guardrails installed along all open side and ends of walking/working platform. Condition occurred on or about 03/31/15.
