29 CFR 1910.303(b)(7)(iv): There was damaged parts that adversely affected safe operation or mechanical strength of the equipment, such as parts that were broken, bent, cut, or deteriorated by corrosion, chemical action, or overheating.  Garage Bay; on or about 31 March 2015 and times prior thereto; a suspended electrical outlet had a damaged faceplate. This condition exposed employees to an electrical shock hazard.

Garage bay; on or about 31 March 2015 and times prior; an energized electrical outlet had a knockout screw plug missing on the bottom of the box.  This condition exposed employees to electrical shock and electrical burn hazard.

29 CFR 1910.303(g)(1)(ii): The required working space about electric equipment rated 600 volts, nominal, or less to ground, was used for storage.  Maintenance facility 12825; on or about 31 March 2015 and times prior thereto; two instances were noted where items  were stored in front of electrical panels:       a. Garage lift bay; a tool box was stored in front of the electrical panel adjacent to the roll-up door.        b. Butler building; various items were stored in front of the electrical panel impeding immediate access to the panel.  This condition exposed employees to delays in de-energizing electrical equipment, electrical shock and electrical burn hazards.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i).  Garage lift bay, work bench area;  on or about 31 March 2015 and times prior thereto; A wall unit switch was installed in lieu of the manufacturers switch on an Ohio Force; 8 inch bench grinder, serial number 102093. This condition exposed employees electrical shock and electrical burn hazards.
