Maintenance facility, Yard area: On or about 31 March 2015 and times prior thereto, above ground 120 gallon propane tank, serial number 940811, was not guarded to prevent vehicular contact. This exposed employees to contact with flammable liquids and other fire hazards.

(b) Maintenance Facility, Lift Bay: On or about 31 March 2015 and times prior thereto, an air nozzle used for cleaning was operating at 100 p.s.i. This exposed employees to laceration/air embolism hazard from using an air nozzle that would not reduce if dead-ended with skin.
