29 CFR 1926.20(b)(2): Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers.  At the time of the inspection, on or about 31 March 2015, the general contractors' foreman and sub-contractors employees were working on two one (1) story roofs of a commercial building located at 2320 Hampton Ave, Saint Louis MO.   The general contractor, or controlling employer, did not conduct a frequent and regular adequate inspection of the job site, materials, and equipment to assure that the ladders onsite, that were being used for accessing the roof, were extended 3 feet above the upper landing for safe access to the roof working areas.
