29 CFR 1926.452(y)(3): Surfaces on which stilts were used were not flat and free of pits, holes, and obstructions, such as debris, as well as other tripping or fall hazards: a. Building A- Employees working from stilts on the third floor were not protected from a fall hazard where debris was allowed to accumulate on the floor where the employees were installing sheetrock. Also multiple extension cords were found on the floor and wrapped around the stilts causing a fall hazard

Building A- An employee was not protected from a fall hazard while standing on the top step of a Werner 4 foot step ladder. The employee was assigned to work on the sheetrock in the ceiling and the employer did not have a stepladder larger than 4 feet at the site.
