29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe condition(s) and the regulation(s) applicable to his work environment to control or eliminate any hazard(s) or other exposure to illness or injury.  Location: Hummock Park Housing Project lot 79  a) Scaffold erected to complete the framing on vaulted ceiling 1st floor  b) 2nd floor, scaffold for framing the ridge of the roof  Employees were exposed to struck-by hazards associated with scaffolding collapse, where they were not trained to recognize or avoid deficient and inadequately constructed scaffolding that had been emplaced in/on the jobsite.

Location:  a) Basement stairwell opening on 1st floor b) Unguarded stairway from 1st to 2nd floor c) Unguarded window on stairway landing d) Unguarded windows on the first floor above unframed foundation hole (3) e) Unframed foundation for porch on the front of the House      Employees were exposed to fall hazards up to 13 feet to the ground while working on a residential framing job where fall protection was not utilized and windows were not guarded
