29 CFR 1904.39(a)(2): Within twenty-four (24) hours after the in-patient hospitalization of one or more employees amputation or an employee's loss of an eye, as a result of a work-related incident, the employer did not report the in-patient hospitalization, amputation, or loss of an eye to OSHA. a) On or about February 17, 2015 or prior thereto, employer did not report to OSHA a work-related finger tip amputation a Extreme Staffing temporary worker within 24 hours of the amputation. The amputation occurred while using Spida Saw #3 in the Truss Manufacturing Area. Quality Truss & Lumber, Inc. was supervising the employee's work activity at the time of the injury. This item was corrected during the inspection.

29 CFR 1904.29(a): Employer did not use OSHA 300, 300-A, and 301 forms, or equivalent forms, for recordable injuries and illnesses. The OSHA 300 form is called the Log of Work-Related Injuries and Illnesses, the 300-A is the Summary of Work-Related Injuries and Illnesses, and the OSHA 301 form is called the Injury and Illness Incident Report. a) On or about February 17, 2015 or prior thereto: the employer did not record an employee's finger tip amputation that occurred in the Truss Manufacturing Area while using Spida Saw #3. The worker was a temporary employee of Extreme Staffing of Idaho at the time of the injury, but his work was being supervised by Quality Truss & Lumber, Inc. Abatement certification is required for this item.
