29 CFR 1926.404(b)(1)(i): The employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites. These requirements are in addition to any other requirements for equipment grounding conductors:  Near the west end of the terminal in a driveway to a lower level entrance to the terminal, there were no ground-fault circuit-interrupters or assured equipment grounding conductor program to protect employees that used 110 VAC Makita jack hammers to break up concrete. Employees were exposed to the hazard of contacting electricity.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not being protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  Near the west end of the airport terminal at a retaining wall for a road next to the terminal, there was no fall protection for employees working and walking next to the wall. Employees were building walls out of CMUs and wrapping up extension cords next to the retaining wall that varied in height from about 28 to 36 inches above grade level. Employees were exposed to the hazard of falling from a height of about 17.75 feet from the top of the retaining wall to the concrete walkway below.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief was provided which will prevent pull from being directly transmitted to joints or terminal screws:  Near the west end of the airport terminal, next to an employee parking lot, and near a retaining wall for a road next to the terminal, the outside insulation on a flexible cord was not connected to the plug so that strain relief was provided. The cord provided power to a 110 VAC Skil cicular saw used to cut lumber. Employees were exposed to the hazard of contacting electricity.
