29 CFR 1910.134(c)(3): The employer did not designate a program administrator who was qualified by appropriate training or experience to administer or oversee the respiratory protection program and to conduct the required evaluations of program effectiveness: (a) East Coast Erosion Blankets, LLC - The employer did not designate a program administrator who was qualified as evidenced by the number of employees who were wearing the SAS Model 8710 disposable half-mask respirators for protection straw dust incorrectly, on or about April 1, 2015. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.134(c)(4): The employer did not provide respirators, training, and medical evaluations at no cost to employees: (a) East Coast Erosion Blankets, LLC - The employer did not provide adequate respirator training for the employees who were wearing the SAS Model 8710 disposable half-mask respirators for protection straw dust as evidenced by the fact that the majority of the employees were wearing the respirators incorrectly, on or about April 1, 2015. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
