29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   On or about April 1, 2015 during the installation of masonry brick at the job site located at 9536 Broadview Rd, Broadview Heights Ohio 44147 the employer did not ensure employees working on the ground were protected from possible danger of head injury from impact, or from falling or flying objects from the employees working directly above on a scaffold .

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports.    On or about April 1, 2015, during the installation of masonry brick at the job site located at 9536 Broadview Rd, Broadview Heights Ohio 44147, the employer did not ensure employees working from a fabricated frame scaffold were protected from falling by ensuring all working platforms were fully planked or decked.

29 CFR 1926.451(e)(9)(iv): Cross braces on tubular welded frame scaffolds were used as a means of access or egress.   On or about April 1, 2015, during the installation of masonry brick at the job site located at 9536 Broadview Rd. Broadview Heights, Ohio 44147 the employer did not ensure employees were provided proper access/egress from a fabricated frame scaffold. Employees were observed gaining access to the working surface by climbing the end frame or a bricklayers scaffold and also the cross braces.

29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person:    On or about April 1, 2015, during the installation of masonry brick 9536 Broadview Rd, Broadview Heights Ohio 44147, the employer did not conduct frequent and regular inspections of the job site where employees were exposed to fall hazards and falling debris from working surfaces of varying heights approximately 9 to 11.10 feet.  Affordable Quality Masonry was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR1926.20(b)(2), which was contained in OSHA inspection number 96884, citation number 01, item number 001a and was affirmed as a final order on 09/13/2011, with respect to a workplace located at 1349 E. 9TH St., Cleveland, OH 44199.

Broadview Heights, Ohio 44147 the employer did not ensure employees were provided proper access/egress from a fabricated frame scaffold. Employees were observed gaining access to the working surface by climbing the end frame or a bricklayers scaffold and also the cross braces.   Affordable Quality Masonry was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR1926.451(e)(1), which was contained in OSHA inspection number 96884, citation number 01, item number 001d and was affirmed as a final order on 09/13/2011, with respect to a workplace located at 1349 E. 9TH St., Cleveland, OH 44199.
