Press Room, Rubber Unload Hopper - The employer did not ensure that all below the hook lifting devices were constructed and load tested to prevent the below the hook lifting devices (i.e. shop made spreader bar used for lifting bags containing 2000 pounds of rubber) from failing which could strike and injure employees.

29 CFR 1910.24(b): Fixed stairs were not provided for access from one structure level to another where operations necessitated regular travel between levels, and for access to operating platforms at any equipment which requires attention routinely during operations:    a.  Press lines 1 and 2 - The employer did not provide a safe means of access to the conveyors, which started and stopped without warning, thereby exposing employees to falls hazards.    b.  Rubber unload area - An employee accessed the rubber unloading deck twelve times per day.  The unloading deck is 3 feet high.  The employee is exposed to a fall hazard.
