Perrysburg, OH: The employer did not have a plan or program to identify job site hazards for employees to maintain a safe working environment. Such a program would allow the employer to identify potential hazards such as falls and struck-by during job site operations, decreasing the chance of employee injury and/or illness.

Perrysburg, OH: The employer failed to ensure employees were protected from falls while installing new roofing material to a residential structure. Employees working in this condition were exposed up to a 16 ft. fall to the ground below.
