29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a)  On November 9, 2016 at the address of 427 Superior Street, Rossford, OH: The employer did not ensure that employees had been trained in the appropriate use of fall protection such as but not limited to the use of personal fall arrest systems or alternative measures while performing various roofing operations from elevated surfaces.

29 CFR 1926.20(b)(1): The employer did not initiate and maintain such programs as may be necessary to comply with this part.  a)  On November 9, 2016 at the address of 427 Superior Street, Rossford, OH: The employer did not have a plan or program to identify job site hazards which assist employees in maintaining a safe working environment.  Such programs would support the employer and employees in identifying and avoiding potential hazards such as falls during job site operations, decreasing the probability of employee injury and/or illness.  Tim Bracey was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.20(b)(1), which was contained in OSHA inspection number 1050611, citation number 1, item number 1a and was affirmed as a final order on May 11, 2015 with respect to a workplace located at 592 Ballbay Ct., Perrysburg, Ohio.

During this activity, employees were exposed to falls up to 18 ft. to the ground below.
