29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. On or about 30 March 2015, and at times prior thereto, the employer did not ensure machine guarding was provided on a Stephan TK 300 S High Speed Dough Mixer at the area where the dough drops out from underneath the mixer onto a conveyor, exposing employees to the hazard of being caught in the rotating mixing baffle.

29 CFR 1910.305(e)(2): Switches, circuit breakers and switchboards installed in wet locations were not enclosed in weatherproof enclosures. On or about 30 March 2015, and at times prior thereto, an electrical snap switch located outside the doorway into the Dough Room had visible damage to the housing in which the conduit had come loose and the internal wires within the housing were visible, exposing workers accessing the switch to an electrical hazard of approximately 120 volts.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws. On or about 30 March 2015, and at times prior thereto, an electrical power cord for a conveyor belt had strain relief damage at the junction box connector in which the internally sheathed wires were visible, exposing employees operating the conveyor to an electrical hazard of approximately 120 volts.
