On or about 30 March 2015, and at times prior thereto, the employer did not ensure machine guarding was provided on a Stephan TK 300 S High Speed Dough Mixer at the area where the dough drops out from underneath the mixer onto a conveyor, exposing employees to the hazard of being caught in the rotating mixing baffle.
