On or about April 02, 2015, employees operating Hitachi pneumatic nail guns were not wearing eye protection while performing roofing activities and exposed to being struck by the flying material and eye injuries.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system.  On or about April 02, 2015, employees performing roofing work on a residential framed structure were exposed to an unprotected fall hazard at a height of over 6 feet to the ground below.
