29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets.   On the west side of the construction site, employees not wearing hardhats while engaged in roofing work below other employees were exposed to struck-by hazards from tools and materials.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:    On the west side of the construction site, employees engaged in roofing activities used pneumatic nail guns and without eye and face protection were exposed to struck-by hazards and eye injuries.   Jose Castillo was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.102(a)(1), which was contained in OSHA inspection number 1050710, citation number 001, item number 1 and was affirmed as a final order on 05/11/2015, with respect to a workplace located at 9729 Amaranth Dr., Fort Worth, Texas 76177.

Jose Castillo was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1050710, citation number 001, item number 2 and was affirmed as a final order on 05/11/2015, with respect to a workplace located at 9729 Amaranth Dr., Fort Worth, Texas 76177.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On the west side of the construction site, employees used a ladder to access and egress the roof that did not extend at least 3 feet above the upper landing surface were exposed to fall hazards of approximately 14 feet.
