29 CFR 1910.23(a)(8)(ii): A floor hole cover of standard strength and construction. While the cover is not in place, the floor hole shall be constantly attended by someone or shall be protected by a removable standard railing.    At the 7646 Beechmont Avenue monopole signs (2) an sign installer was exposed to a 40 foot fall hazard from a floor hole in the wraparound metal work platform while installing a 14' x 48' vinyl sign.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.23(e)(3)(iii): For structural steel railings, posts and top and intermediate rails shall be of 2-inch by 2-inch by 3/8-inch angles or other metal shapes of equivalent bending strength with posts spaced not more than 8 feet on centers.    The Company had not ensured that guardrails at the following locations were constructed to ensure intermediate rail,  and posts spaced at no more than 8 feet on centers.   a. At 3758 Beechmont Leeve, a sign installer was exposed to 40 foot fall hazard from the lack of an intermediate rail on a standard railing on the upper stage metal platform while installing a vinyl sign on a 14' x 48' I beam sign.    b. At 631 Elsinore Place, a sign installer was exposed to 35 foot fall hazard from the lack of an intermediate rail on a standard railing on the upper stage metal platform while installing a vinyl sign on a 14' x 48' I beam sign.    c.  At 5406 Glenway Ave, a sign installer was exposed to a 40 foot and 25 fall hazard from the lack of an intermediate rail on a standard railing on the upper and lower stage metal platform while installing a vinyl sign on a 14' x 48' I beam sign.    d.  At 6500 Colerain Ave, a sign installer was exposed to a 40 foot fall hazard from the lack of an intermediate rail on a standard railing on the upper stage metal platform while installing a vinyl sign on a 14' x 48' I beam sign.    e.  At 618 St. Rt. 32, a sign installer was exposed to a 40 foot fall hazard from a lack of an intermediate rail on an end on a standard railing on the upper stage metal platform while install a vinyl sign on a 14' x 48' I beam sign.    f.  At 8816 St. Rt. 42, a sign installer was exposed to a 40 foot and 25 foot fall hazard from the lack of an intermediate rail on a standard railing on the upper stage and lower stage metal platform while installing a vinyl sign on a 14' x 48' I beam sign. 29 CFR 1910.132(a): Application. Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact.    a.  At 7646 Beechmont Levee billboard, an sign installer employee was exposed to a 40 foot fall hazard while stepping over I beams which obstruct the upper stage work platform without utilizing any fall protection.      b.  At 8816 St. Rt. 42 billboard, a sign installer employee was exposed to a 25 foot fall hazard due to a broken safety line on the front lower stage platform and not maintained in a reliable condition.    c.  At 631 Elsinore Place billboard, a sign installer employee was exposed to a 15 foot fall hazard due to the lack of any fall protection on the rear lower stage platform while installing a vinyl sign.    d.  At 4756 Norwood Lateral billboard, a sign installer employee was exposed to a 25 foot fall hazard due to a broken safety line on the front lower stage work platform while installing a vinyl sign.    e. At 1065 St. Rt. 28 billboard, a sign installer employee was exposed to a 40 foot fall hazard when stepping over I beams which obstruct the upper stage work platform without utilizing any fall protection.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.            Norton Outdoor Advertising, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1007138, citation number 1, item number 1, and was affirmed as a final order on January 12, 2015, with respect to a workplace located at 10814 Hamilton Ave, Cincinnati, OH 45231.
