29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system: (a) 325 Douglas Street Reading, PA - The employee was observed performing roofing work activity on a flat residential roof without the benefit of a fall protection system, thereby exposing employee to a fall of approximately 25 to 26 feet to the ground below, on or about April 1, 2015. No abatement certification or documentation required.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: (a) 325 Douglas Street Reading, PA - Employees were utilizing a Genie S-65 aerial lift to perform roofing work activities and were not protected/restrained inside the basket by the use of a fall harness and attached lanyard, thereby exposing the employees to a possible fall hazard of 25 to 26 feet, on or about April 1, 2015. No abatement certification or documentation required.
