29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: (a) 33 West Market Street York, PA - The employee was observed performing roofing work activity on the steep slope slate roof of a church with a 12 in 12 pitch without the benefit of a fall protection system, thereby exposing the employee to a fall hazard of approximately 30 feet to the ground below, on or about October 17, 2016. No abatement certification or documentation required. McCreesh Slate Roofing LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.501(b)(13), which was contained in OSHA inspection number 1050765, citation number 1, item number 1 and was affirmed as a final order on April 15, 2015, with respect to a workplace located at 325 Douglas Street Reading, PA.
