OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees that were exposed to hazards of rotating inclined conveyors operated on multiple floors that had no audible alarms to warn employees before the conveyors started and no emergency pull cords to stop the conveyors in the event of employees getting caught in or between in-going nip points on the conveyors:   Location: Floors 1 through 4, Four conveyors connected to each other   1. On or about 3/19/15, the employer did not provide an audible alarm to warn employees about the inclined conveyors on four floors of the facility starting. 2. On or about 3/19/15, the employer did not provide emergency stop pull cords along the conveyor for the employees stationed to load, unload, and guide packages to go up the conveyors.
