29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. 385A Canada TOTO Loop, Barrigada: On or about March 31, 2015, there was no fall protection for worker using a pressure washer on the roof, exposing worker to the hazard of falling from a height of approximately 10 feet to the ground below.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access. (a) 385A Canada TOTO Loop, Barrigada: On or about March 31, 2015, right side of single story house, an 18-foot extension ladder did not extend at least three (3) feet above the roof as it extended about 2 to 3 inches above the roof edge, exposing employees to the hazard of falling from a height of approximately 12 feet from the edge of the roof to the ground below.
