29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: a) On or about 4/2/15 at the West Herr Toyota rehabilitation and expansion project located at 4141 Southwestern Boulevard, Orchard Park, New York; employees using 50 foot long ropes with rope grabs attached to a "Trirex" anchor system. The anchor system was positioned approximately 16 feet from the curb and the employees were attached at the end of the 50 foot ropes. Employees were exposed to falls of approximately 21 feet. NO ABATEMENT CERTIFICATION REQUIRED
