29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:     Pasta Production Department, Storci Pasta Machines.   The employer did not ensure that hazardous energy (lockout/tagout) procedures were utilized for an orderly shutdown of the equipment and isolation of hazardous energy during un-jamming and cleaning servicing tasks.  Employees were exposed to caught-in hazards that could result in amputation of arms, hands and fingers.    a)  On or about March 27, 2015, an employee reached into the Storci #3 pasta machine's kneading tank from a side window in order to remove accumulated pasta dough from the inside of the kneading tank. The employer developed and documented a lockout/tagout procedure for the machine that was not utilized for the protection of the employee. The employee was not protected from unexpected start-up and residual motion of the rotating kneader blades while the employee's hand and arm was in the hazardous area.       b) Employees removed the covers of the kneading tanks on Storci pasta machines and reached inside in order to remove accumulated pasta dough from the inner walls of the tanks in preparation for product changeover. The employer developed and documented a lockout/tagout procedure for the machines that was not utilized for the protection of these employees.  Employees were not protected from unexpected start-up and residual motion of the rotating kneader blades while their hand(s) and arm(s) were in the hazardous area.    c) Employees opened covers for the upper level mixers on Storci pasta machines and reached down inside the mixers to remove accumulated dough causing jams inside the mixing tanks.  The employer developed and documented a lockout/tagout procedure for the machines that was not utilized for the protection of these employees.  Employees were not protected from unexpected start-up and residual motion of the rotating mixing blades while their hand(s) and arm(s) were in the hazardous area.      Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.147(d): The established procedures for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  a) Pasta Production Department, Storci Pasta Machines.  Employees assigned to perform un-jamming and cleaning servicing tasks between operating cycles were not required to apply energy controls in the following sequence:  1) turn off and shut down the machines, 2) physically locate and operate energy isolating devices in such a manner as to isolate the machines or equipment from the energy source(s), 3) affix lockout or tagout devices to each energy isolating device, 4) verify that isolation and deenergization of the machines or equipment have been accomplished.  Employees were exposed to caught-in hazards likely to result in amputation of arms, hands and fingers.   Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.147(c)(7)(i)(A): Each authorized employee did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control:     Pasta Production Department.  Authorized employees assigned to perform servicing between normal operating cycles, such as cleaning and clearing pasta dough accumulations, were not trained in established procedures for the safe application and usage of energy controls.          Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d)

29 CFR 1910.335(a)(1)(i): Employees working in areas where there were potential electrical hazards were not provided with electrical protective equipment that was appropriate for the specific parts of the body that needed to be protected and for the work being performed:     Maintenance employees who performed electrical troubleshooting including voltage testing on energized components did not wear electrical protective equipment such as arc resistant apparel, face shields, head and hand protection to protect against electrical shock and arc flash/blast hazards.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
