29 CFR 1910.147(c)(6)(i): The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed. On or about March 21, 2020, the employer failed to perform periodic inspections on the energy control procedure for the Storci Sfogliatrice Sheeter over the past 3 years. This exposed machine operators to caught-in and amputation hazards due to lack of procedure review and employee competency. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) are not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): The employer does not protect employees from the hazardous energy of machinery or equipment by turning off or shutting down machinery or equipment as required by energy control procedures, ensuring energy isolation devices are operated to isolate machinery or equipment from their energy source(s), and affixing lockout devices to energy isolation devices by authorized employees. Employees were exposed to machine hazards from moving parts when they were engaged in servicing and maintenance activities on the Storci Sfogliatrice Sheeter (Pasta Laminator). The employer does not ensure that energy control application steps are implemented prior to allowing employees to clean, remove accumulated pasta dough, or install inner door gaskets. The machine is not shut down or turned off prior to engaging in these activities, exposing employees to caught-in hazards that could result in amputation. The violation most recently occurred on or about March 21, 2020, when an employee was not protected from the hazardous energy of machinery or equipment and suffered an amputation. As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lockout/tagout device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)(i)], and verification of isolation [1910.147(d)(6)] are not implemented to protect employees from machine servicing hazards. Pursuant to 29 CFR 1903.19(d), the employer must submit documents describing the steps it is taking to ensure compliance, including a description of how these steps protect its employees from hazardous energy of machinery or equipment. Rana Meal Solutions, LLC was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1910.147(d), which was contained in OSHA inspection number 1051117, citation number 1, item number 1b, and was affirmed as a final order on April 20, 2016, with respect to a workplace located at 550 Spitzer Road, Bartlett, IL 60103.

29 CFR 1910.147(c)(7)(i)(A): Each authorized employee does not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control. The employer does not ensure that employees are trained and/or adequately trained to control hazardous energy sources on equipment where they perform servicing and maintenance activities. Employees who performed equipment cleaning, removed accumulated pasta dough, and installed inner door gaskets on the Sfogliatrice pasta maker are not trained on the types of hazardous energy sources present and the methods and means necessary to isolate and control them. This exposed employees to caught-in hazards that could result in serious injury. Pursuant to 29 CFR 1903.19(d), the employer must submit documents describing the steps it is taking to ensure compliance, including a copy of amendments to its training program and verification of employee training. Rana Meal Solutions, LLC was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1910.147(c)(7)(i)(A), which was contained in OSHA inspection number 1051117, citation number 1, item number 2, and was affirmed as a final order on April 20, 2016, with respect to a workplace located at 550 Spitzer Road, Bartlett, IL 60103.
