29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.   At the interior wall on the north side of the project on March 31, 2015 and at times prior thereto, safe access was not provided for a wood pole scaffold 5 feet, 2 inches above the concrete floor.  Employees were on the scaffold to lay concrete blocks.  Employees climbed the side of the scaffold to gain access.

29 CFR 1926.701(b): Reinforcing steel.  All protruding reinforcing steel, onto and into which employees could fall, shall be guarded to eliminate the hazard of impalement.  At the project on March 31, 2015, and at times prior thereto, five sections of 3/8 inch diameter reinforcing steel were not guarded to prevent impalement.  The reinforcing steel sections protruded vertically 45 inches from the top level of concrete blocks adjacent to where employees were walking and working.
