29 CFR 1926.404(b)(1)(i): The employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites. These requirements are in addition to any other requirements for equipment grounding conductors: (a) At the project site, on or about April 2, 2015, there were no ground fault circuit interrupters or assured equipment grounding conductor program when employees used a 110 VAC KUNWOO KMC-25H, powered through an extension cord, to cut re-bars.

29 CFR 1926.405(a)(2)(ii)(I): Flexible cords and cables were not protected from damage. (a) At the project site, on or about April 2, 2015, a 110 VAC yellow extension cord was not protected from damage as it lay on the concrete floor and a metal door frame. The cord was exposed to being pinched between the door and the metal frame. Employees were exposed to the hazards of electrical shock and burn.

29 CFR 1926.403(h): Identification of disconnecting means and circuits. Each disconnecting means required by this subpart for motors and appliances was not legibly marked to indicate its purpose, unless located and arranged so the purpose is evident. Each service, feeder, and branch circuit, at its disconnecting means or overcurrent device, shall be legibly marked to indicate its purpose, unless located and arranged so the purpose is evident. These markings shall be of sufficient durability to withstand the environment involved. (a) At the project site, on or about April 2, 2015, employer failed to mark safety switch mounted outdoors to indicate its purpose. The switch was left in ON position.
