29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a)  At Southern Plus Enterprises:  On or about March 31, 2015 and at times prior; the employer failed to ensure that employees working involved in siding activities exposed to fall hazards had been trained on fall protection to include but not limited to the nature of fall hazards in the work area, the role of employees in fall protection plans and on how to install fall protection exposing his employees to a fall hazard of 15 feet to the deck below while working from scaffold platform.  b)  At Southern Plus Enterprises:  On or about March 31, 2015 and at times prior; the employer failed to ensure that employees working involved in siding activities exposed to fall hazards had been trained on fall protection to include but not limited to the nature of fall hazards in the work area, the role of employees in fall protection plans and on how to install fall protection exposing his employees to a fall hazard of 15 feet to the deck below while working from the second level deck.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) At 23344 Perdido Beach Blvd, Orange Beach, Alabama:  On or about March 31, 2015 and at times prior; the employer failed to ensure an extension ladder used by employees to gain access to the scaffold platform had it rails extend at least three feet above the landing area exposing employees to a fall hazard of 15 feet to the deck below.  b) At 23344 Perdido Beach Blvd, Orange Beach, Alabama:  On or about March 31, 2015 and at times prior; the employer failed to ensure an extension ladder used by employees to gain access to the front deck had it rails extend at least three feet above the landing area exposing employees to a fall hazard of 12.7 feet to the ground below.
