29 CFR 1910.106(d)(2)(i): Approved containers and/or portable tanks were not used for the storage of flammable liquids:   (a) Sunset Crater, Maintenance Shop: On or about April 3, 2015 and at times prior thereto, gasoline was stored in an unapproved two-compartment fuel/oil (dolmar-style) propylene container. The standard specification of this container (ASTM F-852) was not a recognized test standard under the Nationally Recognized Testing Laboratory (NRTL) Program.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  (a) Sunset Crater, Chlorination Facility: On or about April 3, 2015 and at times prior thereto, an eye wash station was not provided where bleach was used to disinfect the water supply at the site.   The National Park Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.151(c), which was contained in OSHA inspection number 913241, citation number 1, item number 1 and was affirmed as a final order on 11/12/13, with respect to a workplace located at Wolverton Water Treatment Plant, Sequoia National Park, CA 93262.

29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries:    (a) Sunset Crater, Maintenance Shop: On or about April 3, 2015 and at times prior thereto, access to a wall-mounted Sentry ABC portable fire extinguisher was blocked by equipment used for draining oil.    The National Park Service was previously cited for a violation of this occupational safety and health standard, 29 CFR 1910.157(c)(1), which was contained in OSHA inspection number 952970, citation number 1, item number 1 and was affirmed as a final order on 24 February 2014, with respect to the Lake Mead Recreation Area located at Boulder City, NV 89005.

The National Park Service was previously cited for a violation of this occupational safety and health standard, 29 CFR 1910.215(a)(4), which was contained in OSHA inspection number 994371, citation number 1, item number 6 and was affirmed as a final order on 4 February 2015, with respect to the Grand Canyon, Facility Management Division located at Shuttle Bus Road, Grand Canyon, AZ 86023.

The National Park Service was previously cited for a violation of this occupational safety and health standard, 29 CFR 1910.215(b)(9), which was contained in OSHA inspection number 994371, citation number 1, item number 7 and was affirmed as a final order on 4 February 2015, with respect to the Grand Canyon, Facility Management Division located at Shuttle Bus Road, Grand Canyon, AZ 86023.

The National Park Service was previously cited for a violation of this occupational safety and health standard, 29 CFR 1910.253(b)(4)(iii), which was contained in OSHA inspection number 952287, citation number 2, item number 2 and was affirmed as a final order on 4 February 2015, with respect to the Lake Mead National Recreation Area located at 440 Lake Shore Drive, Boulder City, NV 89005.
