29 CFR 1910.23(a)(3): Every hatchway and chute floor opening was not guarded by one of the methods as specified in paragraphs (a)(3)(i) and (a)(3)(ii) of this section: (a) On or about 03/25/2015 - Browns Ferry Nuclear Plant, Athens, AL, an employee fell approximately six feet into vertical ductwork while inspecting the A blower bank dampers in the drywell.
