29 CFR 1926.853: The employer did not ensure the shoring of floors that were weakened through demolition to carry safely the intended imposed load from demolition operations: a) Jobsite, 605 10th Street, Beaver Falls, PA 15010: Observed on or about March 24, 2015, floors weakened by the demolition process were not shored. A floor of the church steeple was weakened by the removal of joists by another contractor so that the church bell could be removed. The weakened floor collapsed under the weight of three employees and masonry debris. Two of the three employees working on the floor fell a distance of approximately 10 feet to the level below when the collapse occurred.

29 CFR 1926.854(a): Masonry walls, or other sections of masonry, were permitted to fall upon the floors of the building which exceeded the safe carrying capacities of the floor: a) Jobsite, 605 10th Street, Beaver Falls, PA 15010: Observed on or about March 24, 2015, masonry walls and other debris permitted to fall upon a wooden floor of the church steeple exceeded the safe carrying capacity of the floor. Two of the three employees working on the floor fell a distance of approximately 10 feet to the level below when the floor collapsed.
