29 CFR 1926.95(a): Personal protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, or maintained in a sanitary and reliable condition it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation, or physical contact: (a) Jobsite: On or about September 7, 2017, an employee was not wearing a hard hat, eye protection, and flame retardant clothing (jacket and pants) while he was using a cutting torch to cut steel to help demolish a building.

29 CFR 1926.453(b)(2): A safety harness was not worn and a lanyard was not attached to the boom or basket when working from an aerial lift: (a) Jobsite: On or about September 7, 2017, an employee was not wearing any fall protection while he was working out of a aerial lift bucket truck. The fall hazard to the employee was about 25 feet to the ground.
