29 CFR 1910.106(e)(6)(ii): Category 1 or 2 flammable liquids, or Category 3 flammable liquids with a flashpoint below 100 °F (37.8 °C), were dispensed into containers without the nozzle and container being electrically interconnected:    a) Mixing Room:  An employee dispensed a flammable solvent (methyl ethyl ketone) from the main methyl ethyl ketone solvent line into a 5-gallon pail without the nozzle and the container being electrically interconnected; on or about 4/13/15.    b) Mixing Room:  An employee dispensed flammable resin (dimethylformamide) from 3 drums into a mixing tank without the drums and the mixing tank being electrically interconnected; on or about 6/18/15.    c) Mixing Room:  An employee dispensed flammable solvents (toluene and isopropyl alcohol) from the main solvent lines into a 5-gallon pail without the nozzle and the container being electrically interconnected; on or about 6/18/15.

29 CFR 1910.133(a)(1): Protective eye equipment was not required where there was a reasonable probability of injury that could be prevented by such equipment:    a)  Coating Line (C) - Inside Coating Head Stations:   Employees changing drums, totes and inspecting and adjusting coating related equipment during the pumping and application of solvent-based coatings (toluene, methyl ethyl ketone, dimethylformamide, etc.) onto fabric and were not required to wear appropriate eye protection (e.g., chemical splash goggles or equivalent); on or about 3/25/15.    b) Coating Line (C) - Inside Coating Head Stations:   Employees changing drums, totes and inspecting and adjusting coating related equipment during the pumping and application of solvent-based coatings (toluene, methyl ethyl ketone, etc.) onto fabric and were not required to wear appropriate eye protection (e.g., chemical splash goggles or equivalent); on or about 6/18/15.

29 CFR 1910.138(a): The employer did not require employee(s) to use appropriate hand protection when employees' hands were exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasion; punctures; chemical burns; thermal burns; and harmful temperature extremes:    a) Coating Line (C):  An employee moved equipment inside the coating head station (e.g., pump, tote tank, drums, etc.) that had solvent-based coating residue on them (e.g., toluene, dimethylformamide) and was not required to wear chemical-resistant gloves; on or about 4/13/15.    b) Mixing Room - An employee handled equipment (e.g., tote tanks, bonding wires, etc.) that was coated with residue from solvent based coatings (toluene, dimethylformamide),  and was not required to wear chemical-resistant gloves; on or about 4/13/15.    c) Coating Line (C) - An employee handled equipment (e.g., pump, tote tank, etc.) inside the coating head station that had a solvent-based coating residue on it (toluene, dimethylformamide) and was not required to wear chemical-resistant gloves; on or about 6/18/15.

29 CFR 1910.134(h)(3)(i)(A): Respirators used in routine situations were not inspected before each use:  a)  Coating Line and Mixing Room Areas:  Employees mixing solvents (toluene, methyl ethyl ketone, dimethylformamide, etc.) and entering coating head stations used half-face and full-face air purifying respirators that were missing inhalation valves and the respirators were not inspected before each use;  on or about 3/25/15.   Avery Dennison Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard [ 29 CFR 1910.134(h)(3)(i)(A) ] which was contained in OSHA inspection number 329449, citation number 2, item number 1 and was affirmed as a final order on August 23, 2012, with respect to a workplace located at 524 Route 303, Orangeburg, NY 10962.
