29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:    (a)  The employer did not require employees who worked on the bottle filling stations to wear face shields to protect their eyes and faces from splashes of caustic and corrosive chemicals when operators and supervisors were making adjustments to the filling stations.    (b)  The employer's written PPE Hazard Assessment did not require employees who worked with caustic and corrosive chemicals, such as acetic acid, formic acid, and sodium hydroxide, to wear face shields with safety glasses underneath.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.1052(d)(1)(i):  Where Methylene Chloride is present in the workplace, the employer shall determine each employee's exposure by either (A) Taking a personal breathing zone air sample of each employee's exposure; or (B) Taking personal breathing zone air samples that are representative of each employee's exposure.    (a)  The employer did not perform initial monitoring by taking personal breathing zone air samples to determine full shift employee exposure to methylene chloride in the workplace.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.134(f)(2): The employer shall ensure that an employee using a tight-fitting facepiece respirator is fit tested prior to initial use of the respirator, whenever a different respirator facepiece (size, style, model or make) is used, and at least annually thereafter.  (a)  The employer did not ensure that employee(s) who were required to wear a tight-fitting facepiece respirator, such as 3M 6800 full face, 3M 6300 half face and North 7300 half face, while purifying and bottling chemical solvents, were fit tested annually.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
