29 CFR 1910.132(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, or maintained in a sanitary and reliable condition wherever it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact: (a) On May 31, 2016, the employer did not provide protective barriers and shields for employees who were filling and capping one liter bottles of carbon disulfide with the automated crown capping machine, to protect employees from sprays or splashes if the crown capper malfunctioned or the bottle broke. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation: (a) On May 31, 2016, two employees who were operating the bottle filling and crown capping stations were not wearing face shields to protect their eyes and faces from splashes of carbon disulfide, which can cause eye and skin burns, when the crown capper malfunctioned and the bottle being capped broke, spraying the two employees with carbon disulfide. The employer, Tedia Company, Inc. dba Tedia High Purity Solvents was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.133(a)(1), which was contained in OSHA inspection number 1051959, citation number 1, item number 1 and was affirmed as a final order on May 22, 2015, with respect to the workplace located at 100 Tedia Way, Fairfield, OH. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.1052(d)(1)(i): Where Methylene Chloride is present in the workplace, the employer shall determine each employee's exposure by either (A) Taking a personal breathing zone air sample of each employee's exposure; or (B) Taking personal breathing zone air samples that are representative of each employee's exposure. (a) The employer did not use an OSHA approved method to perform initial monitoring by taking personal breathing zone air samples to determine full shift employee exposure to methylene chloride in the workplace. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET)
