29 CFR 1926.503(a)(2)(ii): The employer did not assure that each employee, as necessary, had been trained by a competent person in the correct procedures for erecting, maintaining, disassembling and inspecting the fall protection systems to be used: (a) Location: site On or about 4/6/15, employees were not trained in the procedure for installing Fall Tech swivel anchors model 7451A as part of a fall arrest system. The manufacturer requires a 5/8 inch hole drilled into the concrete a minimum of 5.8 inches from the edge torqued to 85 ft./lbs. The anchors in use on this site were in 7/8 inch holes located 2 inches from the edge of the concrete and could be pulled out by hand with no effort.
