29 CFR 1926.100(a):  Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    (a) Rear of Building - on or about April 7, 2015 - a ground laborer that was working below employees on a scaffold system was not wearing a hard hat.  Employees on the scaffold were installing masonry stone to the exterior of the building.  No other provisions, such as debris nets on the scaffold, were installed.
