29 CFR 1926.304(h)(1): Each circular crosscut table saw shall be guarded by a hood which shall meet all the requirements of paragraph (i)(1) of this section for hoods for circular ripsaws.  R1-Lot 55 Agana Heights Residential Construction Project: The DeWalt DW745 Table Saw (SN: 2013 28-CT251875) was not equipped with a hood guard.  Employees cutting lumber for formwork were exposed to injuries to the hands and upper extremities.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder.  In no case shall the extension be such that ladder deflection under a load would, by itself, cause the ladder to slip off its support.  R1-Lot 55 Agana Heights Residential Construction Project: The Keller Light Duty 16-foot extension ladder used to reach the rooftop formwork did not extend above the upper landing surface and employees were exposed to 9-foot, 8-inch falls to the ground below.

29 CFR 1926.701(b): Reinforcing steel.  All protruding reinforcing steel, onto and into which employees could fall, shall be guarded to eliminate the hazard of impalement.  R1-Lot 55 Agana Heights Residential Construction Project:    a.) Sections of rebar 25 inches from the ground were not protected with rebar caps nor otherwise guarded against the hazard of impalement.  The rebar sections were located in a well travelled area adjacent to the table saw used to cut the formwork being installed at the time of the inspection.  b.) Sections of rebar approximately 16 inches above the roof level at the point where employees get on and off the ladder were not protected with rebar caps nor otherwise guarded.
