29 CFR 1915.134(e): Guards were not mounted as to maintain proper alignment with the wheel in case of accidental breakage. a) Parts/Tool Room - On or about 4/7/2015, the tongue guard on the Sioux Bench Grinder, Model # 2091 was missing exposing employees to struck-by hazards in the event of a bursting wheel.
