DeCicco building -- On or about 4/5/17, 3 extension cords used for crane engine block heaters were observed with abrasion through the outer jacket insulation and damage to the gripped end of the plug.

(b) Fabrication area, outside CNC building: Tubular welded frame scaffolds erected on concrete pad did not bear on base plates.
