29 CFR 1926.451(g)(4)(iv): Midrails were not installed at a height approximately midway between the top edge of the guardrail system and the platform surface: North side residential construction site: On or about April 7,2015, employees were exposed to a fall hazard greater than 10 feet in height to ground level below while installing cornice on an exterior wall from a pump jack scaffold that was missing a midrail.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. North side residential construction site: On or about April 7,2015, employees were exposed to a fall hazard greater than 10 feet in height to ground level below while not being trained on how to recognize the hazards associated with the scaffold use and installing cornice on an exterior wall from a pump jack scaffold that was missing a midrail.
