29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  Building 2:  On or about November 7, 2017, an employee was exposed to struck-by hazards from falling roofing material and tools, while installing fascia siding on an apartment building, when working from an aerial lift without wearing a hardhat.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system:  Building 2: On or about November 7, 2017, an employee without fall protection was exposed to fall hazards, while working from an aerial lift to install fascia siding.  Francisco Guzman was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.453(b)(2)(v), which was contained in OSHA inspection number 122998, citation number 2, item number 1 and was affirmed as a final order on June 15, 2017, with respect to a workplace located at 1639 Cable Ranch Rd, San Antonio, TX 78245.  Francisco Guzman was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.453(b)(2)(v), which was contained in OSHA inspection number 1055929, citation number 1, item number 1 and was affirmed as a final order on June 4, 2015, with respect to a workplace located at 24442 N US HWY 281, San Antonio, TX 78261.  Francisco Guzman was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.453(b)(2)(v), which was contained in OSHA inspection number 1021404, citation number 1, item number 1 and was affirmed as a final order on February 18, 2015, with respect to a workplace located at 1210 North Hunt Lane, San Antonio, TX 78251.  Francisco Guzman was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.453(b)(2)(v), which was contained in OSHA inspection number 1004840, citation number 1, item number 1 and was affirmed as a final order on February 18, 2015, with respect to a workplace located at 7035 Pickwell Dr., San Antonio, TX 78214.
