29 CFR 1915.1001(f)(2)(i): The employer did not ensure that a qualified person conducted an exposure assessment immediately before or at the initiation of operations to ascertain expected exposures in the workplace or during the operation. Vessel Cape Breton, Engine Room: The employer did not conduct an exposure assessment prior to the initiation of work that involved the removal and dismantlement of a desalinization unit. As a result, employees were exposed to asbestos when they cut away and swept up 12% amosite asbestos containing piping insulation.

29 CFR 1915.1001(k)(3)(i): Before work in areas containing ACM and PACM was begun, the employer did not identify the presence, location, and quantity of ACM, and/or PACM, as required by 29 CFR 1915.1001(k)(1). Vessel Cape Breton, Engine Room: The employer did not identify asbestos containing material that employees were exposed to while they worked on the removal and dismantlement of a desalinization unit.
