NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. 29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  (a) On or about April 6, 2015 Ecolab, Inc. were not trained on the chemicals they were introducing into the work site.  They were not trained on how to handle the chemicals safely if spilled.  They were not trained on the hazards of mixing the chemicals with other chemicals and the hazards of the resulting mixtures.   Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.  NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. 29 CFR 1910.1200(h)(3)(iv):   The details of the hazard communication program developed by the employer, did not include an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employee could obtain and use the appropriate hazard information:  (a) On or about April 6, 2015 Ecolab, Inc. provided safety data sheets following an emergency response that did not include all relevant information necessary to determine what chemicals were present and the hazards their presence could have on employees.  (b) On or about April 6, 2015 Ecolab, Inc. had installed a chemical dispersion system to deliver chemicals from drums to the commercial laundry, but had not included a workplace labeling system to inform employees of what chemicals were in the delivery lines.    Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.  NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.
