29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a)    On or about 4/8/2015, The Cell # 97 MAZAK Laser cutter did not have adequate barrier guarding to prevent employee exposure to the robotic material loader.The gate in the barrier guard fencing was not locked or equipped with a control reliable safety interlock.    b)    On or about 4/8/2015, The Cell # 93 MAZAK Laser cutter did not have adequate barrier guarding to prevent employee exposure to the robotic material loader.  The gate in the barrier guard fencing was not locked or equipped with a control reliable safety interlock.    c)    On or about 4/8/2015, The Cell # 91 MAZAK Laser cutter did not have adequate barrier guarding to prevent employee exposure to the robotic material loader. The guardrail around the machine did not adequately prevent entry into the hazardous area.    d)    On or about 4/8/2015, The company had a baler in the paint area of the plant that had a safety interlock that was not operational. This allowed the baler to cycle with the guard gate open.    e)    On or about 4/8/2015, The company had STRIPIT machines that had guard doors which were not interlocked. This allowed the machines to operate in the run mode with the door up in the open position.
