On or about April 08, 2015, at the above addressed jobsite, an employee on an aerial lift (Genie, S-65) was not wearing a body belt or full body harness. The employee was thereby exposed to the hazards associated with a fall of approximately fifteen (15) feet.

On or about April 08, 2015, at the above addressed jobsite, an employee was conducting framing operations with a retractable anchored to an aerial lift (Genie, S-65). The employee's personal fall arrest system did not limit contact from a lower level, thereby exposing the employee to struck by hazards.
