29 CFR 1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards. a) Jobsite - Employees working on a low slope roof were not provided effective training to recognize fall hazards, and techniques to mitigate those hazards, including but not limited to, the trigger height requirement for fall protection systems. Noted on or about 26 MAR 2015.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. a) Jobsite - Employees engaged in roofing preparation operations were not protected from fall hazards at heights of approximately fifteen (15) feet. The building at the widest point was greater than fifty (50) feet. Observed on or about 26 MAR 2015. McMullen Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(10), which was contained in OSHA inspection number 314230590, citation number 1, item number 1 and was affirmed as a final order on 23 FEB 2011, with respect to a workplace located at 235 North 11th Street, Philadelphia, PA.
