29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment: (a) At the north side of the Molding Department, a Sullair air compressor was installed 20 inches in front of electrical disconnect boxes carrying 480 volts AC. In addition, chemical containers were being stored in front of the electrical disconnect boxes, exposing employees to electrical and tripping hazards. (b) At the north side of the Molding Department, a Quincy air compressor was installed 22 inches in front of an electrical disconnect box carrying 480 volts AC, exposing employees to an electrical hazard.

29 CFR 1910.305(g)(1)(iii): Flexible cords and cables were used for purposes prohibited by subparagraphs (A) through (E) of this paragraph: (a) At the north side of the Molding Department, flexible cords were being used as a substitute for fixed wiring. The cords were attached to vertical and overhead metal pipes and portions of the flexible cords were concealed behind electrical panels and metal beams, exposing the employees to an electrical hazard.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: (a) At the north side of the Molding Department, a metal electrical outlet box, designed to be permanently mounted was connected to a flexible electrical cord, and plugged into a mounted electrical outlet box. The metal outlet box was hanging by the flexible cord from the mounted outlet box, exposing employees to an electrical hazard. The outlet box was providing 110 volts AC to automatic drain valves for an Ingersoll Air Dryer and an air compressor tank. M.A. Industries, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.303(b)(2), which was contained in OSHA inspection number 473976, citation number 1, item number 2, and was affirmed as a final order on November 9, 2012, with respect to a workplace located at 303 Dividend Drive, Peachtree City, GA 30269.
