29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: The employer did not ensure that two employees working from an aerial lift were protected from fall hazards. This violation occurred on or about October 15, 2014 at a jobsite located at 5929 E. Northwest Highway Dallas, TX 75220; where two employees who were working from an aerial lift were not wearing body harnesses with a lanyard attached to the boom or basket.
