29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, and rotating parts: (a) On or about April 9, 2015, and times prior thereto, in the Packaging area, employees were not protected from entering into the bagger station on Cortese Boarder Machine #815. (b) On or about March 30, 2015, and times prior thereto, in the Packaging area, employees were not protected from entering into the bagger station on Cortese Boarder Machine #811. Failure to do so exposed employees to contact with moving parts and pinch-points.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) On or about March 30, 2015, in the Packaging area, the employer did not ensure employees utilized lockout tagout procedures prior to servicing and unjamming the bagger on Cortese Boarder Machine #811. Failure to do so exposed employee to contact with pinch-points when the machine unexpectedly energized. (b) On or about April 9, 2015, in the Packaging area, employees did not utilize lockout tagout procedures for the Cortese Boarder Machine #811 prior to working inside the folder making adjustments and aligning the sensor. Failure to do so exposed employees to contact with moving parts and pinch-points if the machine unexpectedly energized. Hanesbrands, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(4)(i) which was contained in OSHA Inspection number 311808877, citation number 1, item number 10 and was affirmed as a final order on March 29, 2011, with respect to a workplace located at 631 Ridgely Street, Suite #1, in Dover, Deleware.
