29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Crossbraces shall not be used as a means of access.  a.  At the construction site located at 393 N. Columbia Avenue, Bexley, Ohio, employees accessed roof bracket scaffolds from a supported scaffold platform that was approximately 4 feet below the point of access, thereby exposing employees to a fall hazard.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers.    a.  At the construction site located at 393 N. Columbia Ave., Bexley, Ohio, employees were performing roofing work from a welded frame scaffold approximately 20 feet in height without fall protection, thereby exposing employees to a fall hazard.    b.  At the construction site located at 393 N. Columbia Ave., Bexley, Ohio, employees were performing roofing work from roof bracket scaffolds that were installed on a roof with a steep pitch and 22 feet in height from the eave, without fall protection, thereby exposing employees to a fall hazard.

29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.     a.  At the construction site located at 393 N. Columbia Ave., Bexley, Ohio, employees were performing roofing work on a residential house with a pitch of up to approximately 15 in 12 and up to approximately 22 feet in height with no fall protection, thereby exposing the employees to a fall hazard.
